Benjamin House Case Management is a service that can help connect individuals with developmental disabilities to needed services and activities within the community. Case Management connects people with professional services as well as natural supports within their home community to address the array of needs that people with developmental disabilities may have. Case management can assist with obtaining benefits for which individuals are eligible. Case management provides linkage, coordination and monitoring of services that the individual is involved in. Case managers work with the individual, their family, and service providers to establish goals that are person centered to provide the frame work for services. Case Management is funded through Medicaid, CAP MR/DD Medicaid and State Funds with no additional cost to the individual or their families. Services are usually available in less than 30 days from the initial contact based on need, staffing, and location.
